Would like your own copy of the LCMS Directory?

Larimer County Medical Society will gladly send you as many copies of our Physicians Directory as you need. There will be a cost of $5 per copy to cover postage and handling if it is mailed.  In order to receive your copy we will need the following information.

We will send you a copy as soon as we receive your check.  Free copies may be picked up at the Information Centers of local hospitals, the Fort Collins Chamber of Commerce, the Loveland Chamber of Commerce, and the Fort Collins Board of Realtors.
